Job summary

Qualifications

• Registered Nurse license in the state of practice.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• 3 years of Nursing experience; outpatient & home infusion preferred.

Responsibilities

• Develops and maintains core therapy and service line admissions through positive referral source relationships.
• Coordinates and communicates discharge planning activities for new patient referrals and appropriate personnel.
• Collaborates with hospital case managers, home health agencies, and physician offices to coordinate care for new referrals.
• Attends and participates in appropriate meetings and training programs as directed by leadership.
• Demonstrates adaptability to patient population, ensuring privacy and clear communication.
