• Ambulatory or primary care nursing experience is preferred.

Responsibilities

• Coordinate centralized nursing workflows and provider support functions.
• Oversee medication refill requests according to protocols.
• Support call center triage workflows and patient communications.
• Review laboratory results, imaging, and clinical notes for providers.
• Develop and maintain nursing workflows, protocols, and processes.
• Lead clinical projects to improve patient care and efficiency.
• Support chronic care, remote monitoring, and other population health programs.
• Train staff on nursing workflows and best practices.
• Collaborate with clinicians and leadership to improve outcomes.
• Travel between clinic locations as needed.
