Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to serve as Clinical Nurse Manager for our Fayetteville, NC habilitation office. The role leads care planning, staff supervision, and collaboration with clients and Case Managers to meet individualized goals.

Office serves clients across ages in Fayetteville area, with Monday–Friday schedule and strong advancement opportunities. Ideal candidates bring 2+ years RN experience, 2+ years with the IDD population, and supervisory exposure.

Qualifications

• 2+ years of RN experience.
• 2+ years of experience working with the IDD population (Qualified Professional).
• Supervisory experience preferred but not required.

Responsibilities

• Lead, inspire and motivate others to provide exceptional care.
• Develop the care plan and oversee its execution.
• Supervise the habilitation tech staff in the field.
• Facilitate new hire orientation.
• Conduct competencies on new and tenured staff.
• Collaborate with client, family, Case Managers, Hab Techs and other teams to implement and evaluate care plans.
• Make regular client visits to evaluate progress and ensure services meet the plan and definitions.
• Evaluate progress of goals identified in the Plan.
• Maintain ongoing communication with Case Managers/Care Coordinators.
• Carry out supervision of Habilitation Technicians as per policy.

This office provides best in class care to clients of all ages throughout Fayetteville area. As a Clinical Manager your day may look like this!

• Lead, inspire and motivate others to provide exceptional care.

• Make regular client visits, as per policy, to evaluate client progress and to monitor that client services are implemented according to the Person Centered/Individual Support Plan and the service definitions
• Evaluate the progress of goals identified in the Person Centered/Individual Support Plan
• Maintain ongoing communication with Case Managers/Care Coordinators
• Carry out supervision of Habilitation Technicians according to policy and procedure and as mandated by state regulation
