Job summary

BAYADA Home Health Care in Lebanon, MO is seeking a Registered Nurse (RN) to step into the Clinical Nurse Manager role at our Habilitation office, serving clients across the Rolla area. You will lead care teams, develop and oversee care plans, supervise habilitation staff, and coordinate with Case Managers and families to achieve person-centered goals while ensuring service definitions are met.

This is a Monday–Friday, salaried position with growth opportunities and a strong focus on quality

Qualifications

• Requires RN license and 2+ years of RN experience.
• Experience with IDD population preferred.
• Supervisory experience preferred but not required.

Responsibilities

• Lead, inspire and motivate care teams to deliver exceptional care.
• Develop and oversee care plans and ensure execution.
• Supervise habilitation staff in the field and conduct trainings.
• Collaborate with Case Managers, families and other teams to meet client goals.
• Complete client visits and monitor progress per plans and service definitions.

Job description

BAYADA Home Health Care is seeking a Registered Nurse (RN) to fill the position of Clinical Nurse Manager for our Lebanon, MO Habilitation office. This office provides best in class care to clients of all ages throughout Rolla area.

As a Clinical Manager your day may look like this!

• Lead, inspire and motivate others to provide exceptional care.
• Develop the care plan and oversee its execution.
• Supervise the habilitation tech staff in the field.
• Facilitate new hire orientation.
• Conduct competencies on new and tenured staff.
• Collaborate with the client, family, Case Managers, Hab Techs and other teams to implement and evaluate client care needs and individual goals identified in the Person Centered/Individual Support Plan
• Make regular client visits, as per policy, to evaluate client progress and to monitor that client services are implemented according to the Person Centered/Individual Support Plan and the service definitions
• Evaluate the progress of goals identified in the Person Centered/Individual Support Plan
• Maintain ongoing communication with Case Managers/Care Coordinators
• Carry out supervision of Habilitation Technicians according to policy and procedure and as mandated by state regulation

• Registered Nurse (RN) with 2 or more years of experience as an RN.
• 2+ years of experience working with the IDD Population (Qualified Professional)
• Supervisory experience preferred but not required
