Job summary

Constellation is seeking a Hospice RN Clinical Liaison to serve the Philadelphia area. The RN will plan, coordinate and deliver home care services, building relationships with hospitals, skilled nursing facilities, physician offices and community partners to expand hospice referrals.

Requirements include RN license, CPR, state licensure, 1–2 years acute care; Bachelor's preferred; strong communication and organizational skills. Travel to partners and on-call duties are part of the role.

Qualifications

• Graduate of an accredited school of nursing.
• Current state licensure and CPR certification.
• Bachelor’s degree with one year home health care preferred.
• Excellent communication and problem-solving skills.

Responsibilities

• Assess patient and family to determine home care needs.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments as prescribed by physician.
• Coordinate patient care and act as Case Manager when assigned.
• Build relationships with hospitals, SNFs, physicians and community partners to generate referrals.
• Participate in on-call duties and ensure equipment/services are available.
