Job summary

Constellation is seeking a Part Time RN Clinical Liaison in Lancaster, PA to expand hospice and home care referrals. You will build relationships with hospitals, SNFs, physician offices and community partners while delivering patient-centered care.

The ideal candidate is an experienced registered nurse with strong communication, sales/marketing experience, and the ability to plan and coordinate care across teams. This role is on-site and involves occasional on-call duties.

Qualifications

Must be a graduate of an accredited nursing program and licensed in the state.
Current CPR certification and valid RN license.
One to two years of acute care experience; home health preferred.
Excellent communication, problem-solving and planning skills.

Responsibilities

Provides patient care and completes initial assessments in home care settings.
Develops and revises care plans with patient/family involvement.
Coordinates with physicians, social workers, and facilities to arrange services.
Acts as Hospice & Palliative Care Liaison to expand referrals.
Maintains CRM records and documents visits every 60 days.
Participates in on-call duties as defined by policy.
