Job summary

Gentiva Hospice in Flint, Michigan seeks a Registered Nurse Clinical Liaison to manage intake and referrals, coordinate admissions, and support community awareness of hospice services. The role focuses on building strong professional relationships, guiding families, and ensuring compliant, timely patient eligibility assessments.

Ideal candidates have 3–5 years hospice experience (BSN preferred) and an active RN license in Michigan.

Qualifications

• Licensed RN with 3–5 years hospice experience required.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience total.
• Prior discharge planning, case management, or educator experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians and families.
• Educate patients and families on hospice care, processes, and expectations with clarity and compassion.
• Coordinate intake and admission, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and hospice team with clear communication and rapid response.
• Make daily visits to hospitals and facilities to stay accessible to discharge planners and physicians.
• Build and maintain relationships with referral partners and identify outreach opportunities.
