Job summary

Transitions At Home, Inc. is seeking an experienced RN Clinical Administrator to lead the clinical and daily operations of our Home Health Agency in Wisconsin.

This hands-on leader will build strong clinical teams, ensure regulatory compliance, and drive growth while delivering safe, high-quality patient care. The role focuses on leading clinical services, improving patient outcomes, and aligning staff performance with agency goals, all while maintaining patient-centered care within a compliant,

Qualifications

• Current, unrestricted Wisconsin RN license.
• Meets Wisconsin requirements to serve as Home Health Agency Administrator.
• Home health clinical experience preferred.
• Nursing leadership/management/supervisory experience preferred.
• Knowledge of Medicare Home Health COP and Wisconsin regulations preferred.
• Strong leadership, communication, organizational, and problem-solving skills.
• Ability to manage clinical quality while supporting staffing and operations.
• BSN preferred but not required.

Responsibilities

• Provide leadership and oversight of home health clinical operations and staff.
• Ensure compliance with Wisconsin regulations, CMS COP, and agency policies.
• Oversee referrals, admissions, plans of care, transfers, and discharges.
• Supervise RNs, LPNs, Home Health Aides, therapists, and other clinical personnel.
• Monitor clinical documentation, patient outcomes, hospitalizations, and quality indicators.
• Lead QAPI, survey readiness, compliance, and performance improvement activities.
• Recruit, orient, coach, evaluate, and develop clinical staff.
• Monitor staffing, scheduling, productivity, and resource utilization.
• Address patient complaints and employee performance issues.
• Collaborate with physicians, referral sources, and agency leadership.
• Support census growth, referral conversion, retention, and overall agency performance.
• Promote a culture of accountability, teamwork, quality, and patient care.
