Job summary

Wheeling Hospital is seeking a Clinical Coordinator for the 6th floor to coordinate nursing care, implement the nursing process, and collaborate with patients, families, and health-team members to ensure high-quality care.

The role requires an active RN license (state or eNLC) and BLS certification within 30 days of hire, with BSN preferred. At least one year of healthcare experience is desired.

Qualifications

• Current Registered Nurse license issued by the state where services will be provided or multi-state license viaeNLC.
• Basic Life Support (BLS) certification obtainable within 30 days of hire.

Responsibilities

• Collects, documents and analyzes patient health status and medical history; completes admission assessment.
• Formulates and initiates plan of care based on patient needs and sets care priorities.
• Monitors and documents effectiveness of interventions; adjusts care plan as needed.
• Provides education to patient and family; documents teaching initiation and completion.
• Administers medications using two identifiers and documents effectiveness.
• Assists with discharge planning and coordinates care transitions.
• Supports credentialing surveys and PI initiatives; ensures safety and compliance.
• Collaborates with staff and management to optimize patient care and staffing.
