Job summary

Pacific Health Group is seeking a Registered Nurse – Clinical Case Consultant to lead clinical guidance for the CalAIM Enhanced Care Management program. The role supports interdisciplinary teams, oversees LVN supervision, and ensures high-quality, coordinated care for high-risk Medi-Cal members.

The position is hybrid in California, reporting to the ECM Program Manager. Competitive compensation ranges $85,000–$95,000 with full benefits, PTO, and professional growth opportunities.

Qualifications

• Active and unrestricted RN license in California.
• Minimum two years of direct clinical experience in community health, managed care, acute care, or behavioral health.
• Experience working with Medi-Cal populations and individuals with complex needs.
• Experience guiding interdisciplinary care teams is preferred.

Responsibilities

• Serve as the primary clinical resource for ECM care teams.
• Provide clinical consultation to care team members and supervisors.
• Review complex cases and advise care planning and risk mitigation.
• Participate in interdisciplinary case reviews and care team meetings.
• Support field-based supervision and clinical oversight of LVNs and CHWs.
• Monitor field operations for quality and regulatory compliance.
• Coordinate care transitions after hospital or emergency events.
• Support documentation and compliance with Medi-Cal ECM standards.
