Job summary

Bienvivir All-Inclusive Senior Health in El Paso, TX is seeking a Registered Nurse - Clinic (Participant Manager) to oversee nursing assessments, care planning, and coordination of participant care in the clinic and in patients' homes. You will collaborate with the interdisciplinary team, monitor education and disease management, and assist with admissions, discharges, and follow-up visits.

Responsibilities include triage, documentation, and ensuring continuity of care while maintaining quality

Qualifications

• A graduate of an accredited nursing program with TX RN license.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Complete nursing assessments and care plans for participants.
• Coordinate participant care with PCP/IDT and document in medical records.
• Triage calls and monitor participants in clinic and at home visits.
• Coordinate hospital/nursing home admissions and transfers.
• Participate in team meetings and on-call rotation.
