Job summary

Bienvivir All-Inclusive Senior Health in El Paso, TX is seeking a Nurse Participant Manager to coordinate comprehensive care for frail seniors. You will perform nursing assessments, develop care plans with the IDT and PCP, and ensure timely documentation in medical records.

The role includes home visits and triage for participants who are ill or unable to attend the center. Under Clinic Manager supervision, you will work with caregivers and the care team to maintain 24-hour service delivery,

Qualifications

• Registered Nurse licensed in Texas.
• One year of experience with frail or elderly populations preferred.
• Experience coordinating care with PCPs and the IDT.

Responsibilities

• Complete initial nursing assessments within ten days of enrollment.
• Perform routine reassessments and update care plans as needed.
• Develop and document nursing care plans with IDT and PCP.
• Function as triage nurse and contact participants about missed visits.
• Collaborate with PCP to manage flow in exam rooms and visits.
• Conduct home visits for Start of Care assessments and follow-ups.
