Job summary

Bienvivir All-Inclusive Senior Health in El Paso, TX is seeking a Registered Nurse for clinic-based participant management. The role covers nursing assessments, care planning, health education, and coordination of care in clinic and participants’ homes.

The position runs Monday–Friday, 8:00 a.m. to 5:00 p.m., with opportunities to lead care coordination and participate in team activities. Requires Texas RN licensure and experience with frail seniors; bilingual in English/Spanish preferred.

Qualifications

• Graduate of an accredited nursing program with Texas RN license.
• One year of experience with frail or elderly population preferred.
• Bilingual English/Spanish.

Responsibilities

• Conduct initial and routine nursing assessments of participants, including medication adherence.
• Develop and update nursing care plans with the IDT and PCP.
• Coordinate hospital, nursing home and assisted living admissions, discharges and transfers.
• Provide education and counseling to participants and caregivers; document in medical records.
