Live Well Homecare Agency, LLC is seeking a Registered Nurse for the Central Resource Team to provide professional nursing care, assess patient conditions, and coordinate care across the team. The role requires an active Illinois RN license, CPR and ACLS within one year, and a BSN preferred with inpatient RN experience.

• Assess patient conditions, recognize changes, plan, implement, and evaluate care.
• Monitor patient progress and document observations accurately.
• Delegate appropriate tasks to staff and collaborate as part of a healthcare team.

Qualifications:

• Bachelor’s Degree or higher in Nursing preferred; minimum of 1+ years’ RN experience in inpatient settings required.
• Associate Degree in Nursing acceptable if a Bachelor’s Degree is obtained within five years.
• Current Illinois RN license, CPR certification, and ACLS within one year of employment.
