Live Well Homecare in Naperville, IL seeks a Registered Nurse for the Central Resource Team to provide expert nursing care and coordinate patient plans. You will assess conditions, document progress, and ensure adherence to hospital policies in an inpatient setting.

The role requires an active Illinois RN license, 1+ year inpatient RN experience, CPR certification, and ACLS within one year of starting. Bachelor's degree is preferred or must be pursued within five years if ADN holder.

Responsibilities:

• Provide professional nursing care in accordance with state laws, hospital policies, and procedures.


• Assess patient conditions, recognize changes, and plan and evaluate care.


• Monitor patient progress and document observations accurately.


• Delegate appropriate patient care tasks to team members.


• Collaborate effectively within the healthcare team to meet unit goals.

Qualifications & Skills:

• Minimum 1+ years' RN experience in an inpatient setting.


• Associate Degree in Nursing required; Bachelor’s Degree preferred or must be obtained within five years if only an associate degree.


• Current Illinois RN license.


• CPR certification from AHA or BLS.


• ACLS certification within one year of employment start date.
