Job summary

Live Well Homecare in Naperville, IL is seeking a Registered Nurse for the Central Resource Team to provide professional nursing care and coordinate patient services. You will assess conditions, plan and implement care, and collaborate with the healthcare team.

Qualifications

• Bachelor’s Degree in Nursing is preferred; 1+ years RN inpatient experience required.
• ADN acceptable if BSN obtained within five years.
• Current Illinois RN license, CPR and ACLS within one year of employment.

Responsibilities

• Provide professional nursing care following state laws, hospital policies, and procedures.
• Assess patient conditions, recognize changes, plan, implement, and evaluate care.
• Monitor patient progress and document observations accurately.
• Delegate appropriate tasks to staff and collaborate as part of a healthcare team.
