Job summary

myPlace Health in California is seeking a CDI Specialist to ensure accurate, clinically specific provider documentation across the participant population within the PACE model. You will collaborate with physicians, nurse practitioners, and care teams to support compliant CMS-HCC capture and documentation improvement.

Requirements include active, unrestricted RN or NP license in California and at least three years of outpatient clinical experience.

Qualifications

• Active, unrestricted RN or NP license in California.
• Minimum of three years of clinical experience in outpatient/ambulatory settings.
• Foundational knowledge of ICD-10-CM coding principles.
• Strong written and verbal communication skills.
• Proficiency with electronic health record systems and CDI tools.
• Ability to work independently and manage chart review workloads.
• Experience in CDI, risk adjustment, HCC coding, or documentation improvement.
• Familiarity with Medicare Advantage, PACE, capitated or value-based care models.
• CRC/CDEO/CDDS-O/CDIP or commitment to obtain certification within 12 months.

Responsibilities

• Conduct prospective, concurrent, and retrospective reviews of medical records to identify documentation gaps and opportunities.
• Apply clinical reasoning across EMR, labs, imaging, and medications to support accurate documentation.
• Review longitudinal chart trends to identify disease progression and complexity.
• Formulate compliant queries with clinical evidence to support provider clarification.
• Partner with physicians and care teams through education to improve CMS-HCC capture.
• Participate in interdisciplinary meetings to surface documentation opportunities.
• Track documentation patterns and outcomes using CDI tools to support monitoring and audit readiness.
• Develop provider-facing education, templates, and workflow guides aligned with CMS standards.
• Escalate deficiencies and systemic trends to CDI leadership.
