Job summary

Indian Health Board of Minneapolis is seeking a Registered Nurse Certified Diabetes Care and Education Specialist (CDCES) to oversee diabetes education and care coordination in our community clinic. The role ensures NSDSMS standards are met and ADA Recognition is maintained, with weekday scheduling and a full-time, non-exempt status.

The position requires a Minnesota RN license and CDCES certification, with experience in health education and care coordination, preferably in American Indian

Qualifications

• Minnesota RN license required.
• CDCES certification required; candidates on-track may be considered.
• 2+ years of care coordination and health education preferred.
• Experience working in American Indian communities preferred.

Responsibilities

• Act as Health Care Home Care Coordinator to understand and meet patient needs with respect and care.
• Link patients with internal and external providers to improve coordination of services and information flow.
• Develop and maintain individualized clinical care plans and update as needed.
• Provide diabetes education and care coordination in the clinic setting.
• Support ADA NSDSMS recognition and related accreditation activities.
