Job summary

HealthPartners is seeking a Diabetes Care and Education Specialist to join our Lakeview Hospital team. This role supports diabetes care across the lifespan, providing direct care, education, and self-management support to patients, in alignment with licensure and professional standards.

The DCES collaborates with clinicians and uses evidence-based strategies to improve health outcomes, while maintaining patient confidentiality and rights in a fast-paced ambulatory setting.

Qualifications

• RN or RDN with advanced knowledge in diabetes care and education.
• Current RN license in MN or WI or CDCES certification eligibility.
• BLS certification for health professionals required.

Responsibilities

• Provide direct care, education, and support across the lifespan for people with diabetes.
• Promote self-management and goal achievement through evidence-based care planning.
• Collaborate with clinicians to deliver integrated, person-centered care.
• Maintain patient confidentiality and uphold professional standards.
