Job summary

Lakeview Hospital in Minnesota seeks a Lead Diabetes Care and Education Specialist (DCES) to guide daily work across the Diabetes Care and Education Service Line. The role combines direct patient care, education, and program leadership for diverse ages with diabetes.

Qualified candidates are an RN or RD with CDCES certification or eligible within 24 months, plus licensure and BLS. Strong communication and clinical judgment are essential in delivering person-centered care and team collaboration.

Qualifications

• Bachelor's degree in nursing or nutrition and/or Dietetics with 2 years of prior diabetes care and education experience, or
• Associate's degree in nursing with 4 years of prior diabetes care and education experience.
• Demonstrated leadership skills
• Maintain Minnesota RN licensure or Dietitian licensure (RDN) and current MN licensure; WI RN/ND licensure process upon hire

Responsibilities

• Provide direct care, education and support across the lifespan for individuals with diabetes.
• Promote self-management, reduce risks and improve health outcomes.
• Work with the Diabetes Care and Education Service Line to deliver integrated, person-centered care.
