ScionHealth’s Kindred Hospital Chicago - Northlake is seeking a Wound Care Nurse to provide specialized wound, ostomy, and skin care. The role includes assessment, care planning, treatment implementation, education, and meticulous documentation in line with clinical standards.

You will work within a multidisciplinary team to optimize patient outcomes and reduce skin breakdown risks. The position requires active RN licensure, WOCN/CWS/WCC certification within 12 months, BCLS adherence, and 2+

Qualifications

• RN license in state of practice required at hire.
• WOCN/CWS or WCC certification within 1 year of hire required.
• BCLS (Basic Life Support) within facility-specified time frame on hire.

Responsibilities

• Provide comprehensive wound assessments and collaborate with providers on treatment plans.
• Implement and monitor wound care interventions including negative pressure therapy and dressing changes.
• Coordinate care for patients with complex wounds and ensure continuity through discharge planning.
• Educate patients, families, and staff on wound care techniques and prevention.
• Identify patients at risk for skin breakdown and implement prevention protocols.
• Serve as clinical resource for skin integrity and wound management.

Kindred Hospital Chicago - Northlake is a 94-bed long-term acute care hospital offering the same in depth care you would receive in a traditional hospital, but for an extended recovery period. We partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. We are located in the city community offering a nice balance between residential and commercial area, close to Belmont Gardens.

Job Summary

The Wound Care Nurse is responsible for providing specialized nursing care to patients with wounds, ostomies, or skin integrity concerns. This role includes assessment, development of care plans, implementation of treatments, patient and staff education, and documentation in accordance with clinical standards and facility policy. The nurse acts as a resource and consultant to clinical teams regarding complex wound care management and best practices.

Essential Functions

• Provides comprehensive wound assessments and collaborates with providers on treatment plans.
• Implements and monitors wound care interventions including negative pressure therapy, dressing changes, and offloading techniques.
• Coordinates care for patients with complex wounds and ensures continuity through discharge planning and follow-up referrals.
• Educates patients, families, and staff regarding wound care techniques and prevention strategies.
• Identifies patients at risk for skin breakdown and oversees implementation of prevention protocols.
• Serves as a clinical resource and advisor for skin integrity, wound management, and product utilization.
• Participates in performance improvement initiatives and monitors outcomes related to wound care.
• Maintains documentation according to regulatory and organizational standards.
• Collaborates with interdisciplinary team members to ensure integrated care.
• Provides input on wound care supplies and equipment.

• Demonstrated clinical expertise in wound and skin care.
• Strong communication, assessment, and critical thinking skills.
• Knowledge of evidence-based practices in wound healing.
• Ability to educate and mentor staff on wound care protocols.
• Proficient in EMR systems and documentation.
• Capable of lifting 50 lbs. and performing physical tasks required in patient care.

Pay Range: $45.00-$66.87/hr.

Qualifications

Education

• Postsecondary (Cert/Diploma/Program Grad) from an accredited school of nursing (Required) And
• Bachelor’s Degree in nursing or related field (Preferred)

Licenses/Certifications

• RN - Registered Nurse - State Licensure and/or Compact State Licensure in the state of practice Upon Hire (Required) And
• WCC - Wound Care Certified WOCN, CWS or WCC within 1 Year (Required) And
• BCLS - Basic Life Support within Facility-Specified time frame Upon Hire

Experience

• 2+ years nursing experience in acute care, skilled nursing, outpatient, or home health settings (Required) And
• Prior Experience in wound care (Preferred)
