Nightingale Nursing & Rehabilitation Center is seeking an experienced RN Wound Care Nurse to join our clinical team. You will assess wounds, develop treatment plans, and provide care to residents in a skilled nursing environment.

This role requires collaboration with physicians, nursing staff, therapy and dietary teams, and on-call availability. We offer competitive pay, comprehensive benefits, PTO, and opportunities for professional development.

Qualifications

• Active RN license in Pennsylvania.
• Wound care experience in SNF/long-term care preferred.
• Wound Care Certification (WCC/WOCN) preferred, but not required.
• Strong assessment and documentation skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Excellent communication and organizational abilities.
• Commitment to providing compassionate, resident-centered care.

Responsabilités

• Conduct comprehensive wound assessments and ongoing evaluations of resident skin integrity.
• Develop, implement, and monitor individualized wound care treatment plans.
• Perform wound treatments and dressing changes in accordance with physician orders and clinical protocols.
• Monitor and document wound progress, interventions, and outcomes accurately and timely.
• Collaborate with physicians, nursing staff, therapy, and dietary teams to optimize healing outcomes.
• Provide education and guidance to residents, families, and staff regarding wound prevention and treatment.
• Participate in quality assurance and infection prevention initiatives related to wound management.
• Ensure compliance with state, federal, and facility regulations regarding documentation and care delivery.
• Serve as a clinical resource for pressure injury prevention and skin care management.
• Participate in the facility's on-call rotation schedule as assigned.
