Job summary

Live Well Homecare Agency, LLC is seeking an RN Case Manager to provide care coordination and case management for injured team members and facilitate return-to-work.

The role requires a Bachelor's degree in Nursing or Healthcare/Science, 3+ years of clinical nursing experience, an Illinois RN license, and BLS certification. You will collaborate with carriers, TPAs, providers, and internal teams to manage claims and promote safe, compliant care.

Qualifications

• Bachelor’s degree in Nursing or Healthcare/Science is required.
• 3+ years of clinical nursing experience.
• Valid RN license in Illinois.
• BLS certification.

Responsibilities

• Monitoring medical progress through assessments via various visit methods.
• Communicating with carriers, third-party administrators, and providers to manage claims.
• Developing individualized return-to-work plans in collaboration with relevant departments.
• Ensuring compliance with treatment protocols and documentation within case management systems.
• Building relationships with team members, leadership, and external partners.
• Staying current on regulations, best practices, and promoting organizational health initiatives.
• Supporting patient safety, infection control, and confidentiality standards.
