Alternate Solutions Health Network is seeking a Registered Nurse Case Manager in Lexington, KY. The RNCM will lead patient assessments, develop plans of care, and coordinate multidisciplinary care under physician direction.

The role requires active RN licensure and at least one year of acute care experience; home care background is preferred. You will educate patients and families, document thoroughly, and participate in on-call rotations to ensure seamless patient care.
