Job summary

Interim HealthCare Inc. in Kentucky is seeking an experienced Registered Nurse Case Manager (RNCM) to deliver patient-centered home health services. You will conduct in-person assessments, coordinate care with RNs, LPNs, and aides, educate families, and promote continuity of care to reduce readmissions.

Qualifications

• Current, unrestricted RN license to practice in the state of the position.
• CPR/AED/BLS/First Aid certification required.
• Reliable transportation to care sites and/or work locations.
• At least 1 year RN experience in home health or similar setting; case management experience preferred.
• OASIS experience preferred.
• Practical trach/ventilator experience preferred, not required.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee implementation and ongoing assessment of the patient’s plan of care with home health staff.
• Communicate patient conditions and coordinate with providers to adjust care.
• Educate patients and families on home health procedures (wound care, IV, medications).
• Work to decrease readmissions by promoting preventative care and continuity of care.
