CVS Health is seeking a Case Manager RN in Illinois to coordinate care for members with complex medical and social needs. You will liaise with families, providers, and health plans, and manage comprehensive assessments to drive improved health outcomes.

The role requires 3–5 years of clinical nursing and 2–3 years in case management or related areas, with an IL RN license. This is a full‑time position based in Lincoln, IL, serving Logan County and surrounding areas.

Qualifications

Associate or Bachelor's degree in nursing (BSN preferred).
3-5 years of clinical nursing experience.
2-3 years in case management, discharge planning, care coordination, or home health.
Active and unrestricted RN license in IL; CCM certification preferred.

Responsibilities

Serve as liaison among members, families, providers, health plans, employers, and community organizations.
Coordinate and manage care activities for members with complex health conditions.
Perform comprehensive assessments of medical, functional, psychosocial, and environmental needs.
Develop, implement, and monitor individualized care plans to improve outcomes.
Facilitate access to healthcare services and community resources.
Engage with members via telephonic and in-person visits as needed.
Collaborate with interdisciplinary teams to ensure coordinated care.
