Job summary

Abrazo Health is seeking an experienced Registered Nurse Case Manager at the Abrazo Scottsdale Campus in Arizona. You will coordinate care, assess transition needs, and support discharge planning to optimize patient outcomes and resource utilization.

The role emphasizes adherence to standards, collaboration with physicians and teams, and education for patients and families to support post-acute needs.

Qualifications

• Two years of acute hospital patient care experience.
• Graduate of an accredited nursing program.
• Active RN license in state of practice.
• Preferred: experience in hospital case management.
• Preferred: bachelor’s or master’s degree in nursing.
• Preferred: Accredited Case Manager (ACM) certification.

Responsibilities

• Facilitate care along a continuum through resource coordination.
• Assess transitions to promote timely throughput and safe discharge.
• Coordinate care with physicians, nurses, and ancillary staff.
• Lead multidisciplinary patient care conferences.
• Document care plans and ensure compliance with regulatory standards.
