Qualifications

• Registered Nurse with active and unrestricted RN licensure in their state of residence.
• Must have 3+ years clinical practice experience.
• Bachelor’s degree preferred; Associate’s degree required.
• Compact RN licensure preferred; case management experience a plus.
• Proficient with Windows and Microsoft Office; strong computer skills.

Responsibilities

• Assess, plan, implement and coordinate all case management activities with members.
• Evaluate medical needs and facilitate wellness and benefit plan eligibility.
• Use clinical tools and data to address needs and optimize outcomes.
• Assess barriers, comorbidities, and work capacity; coordinate referrals as needed.
• Collaborate with supervisor and team; present cases to multidisciplinary groups.
• Ensure compliance with regulatory and company policies in case management.
