Qualifications

• Bachelor's Degree is required.
• Master's Degree may be preferred for advanced roles.
• RN in New Mexico or licensure is described in credentials.

Responsibilities

• Coordinate initial review, triage and assignment of patients to case managers.
• Supervise staff and provide resources for multidisciplinary care teams.
• Oversee and assign case loads and staff hours to meet hospital needs.
• Conduct timely discharge planning and coordinate resources for patient care.
