Job summary

UNM Hospital is seeking a Care Manager with leadership responsibilities to coordinate, implement and evaluate care management services. The role covers a range of patient ages and requires collaboration with medical staff, program development and quality assurance activities.

The candidate should have a Bachelor's Degree and NM RN licensure with 2 years related experience; Case Manager Certification within 18 months is expected. Relocation and on-site days shift support may be available.

Qualifications

• Bachelor's Degree required.
• RN licensure in NM or as allowed by NM.
• Case Manager Certification within 18 months of position.
• 2 years directly related experience.
• CPR for Healthcare/BLS within 30 days.

Responsibilities

• Coordinate care management delivery and monitoring of quality.
• Develop and evaluate protocols, policies and standards of care.
• Provide clinical supervision to RN Case Managers and staff as needed.
• Participate in orientation and continuing education for care management staff.
• Ensure adherence to hospital policies and safety standards.
• Collaborate with medical staff and other departments to optimize patient care.
• Assist in coordinating external requests for presentations and site visits.
