Job summary

Carolina Family Health Centers, Inc. is seeking a Nurse Case Manager for the Ryan White Program at Freedom Hill Community Health Center in Tarboro, NC. The role focuses on providing medical and non-medical case management within a community health setting.

Under RN supervision, you will conduct AWVs, manage patient care plans, and assist patients in navigating care and medications. This 40-hour/week position includes a full benefits package and bi-weekly pay.

Qualifications

• Graduate of an accredited school of nursing as a Registered Nurse.
• Current and unrestricted RN license in North Carolina.
• Experience in community health or case management preferred.
• Strong communication and interpersonal skills.
• Proficiency with Windows-based environments and EHR systems.

Responsibilities

• Provide medical and non-medical case management to people living with HIV.
• Perform Medicare Annual Wellness Visits (AWV).
• Develop and update individualized, person-centered care plans with patient/family input.
• Assist patients with transitions from ED/hospital to primary care, including med reconciliation.
• Educate patients on preventive care, chronic conditions, medications, and lifestyle recommendations.
• Aim to improve overall quality metrics by closing gaps in care.
