Job summary

Carolina Family Health Centers, Inc. is seeking a Case Manager II (Registered Nurse) for Population Health Medicare. The role supports a panel of patients across Wilson Community Health Center, Harvest Family Health Center, and Freedom Hill Community Health Center.

The position offers a 32‑ or 36‑hour workweek with no weekends, a full benefits package, and bonuses. Travel between sites is required, with a focus on quality, person-centered care.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse license to practice in North Carolina.
• BLS Certification required.
• Must have personal vehicle for frequent travel between centers and patient homes.

Responsibilities

• Provide medical case management to medium/high-risk patients.
• Perform Medicare Annual Wellness Visits (AWV).
• Collaborate with community agencies for referral resources and NC Care 360.
• Establish and document a care plan with patient and family members; update as needed.
• Identify patients with recent ED visits or hospitalizations and assist transitions to primary care.
