Job summary

Support, Inc. is seeking a Nursing Coordinator/Registered Nurse to oversee medical care for individuals with intellectual and developmental disabilities.

You will coordinate with physicians and interdisciplinary teams to promote health outcomes, develop care plans, and ensure compliance with applicable guidelines. The role is part‑time or full‑time, primarily work from home with occasional travel to Aurora, Colorado Springs, Pueblo, and other front-range locations.

Qualifications

• RN with current Colorado license.
• Experience with individuals with intellectual and developmental disabilities preferred.
• Nurse case management experience preferred.

Responsibilities

• Provide delegation and consultation to unlicensed providers, families, and Support Inc staff.
• Provide medical consultation to interdisciplinary team members
• Teach and train providers, families, and staff in: Medication Administration (QMAP) and MRB training; Individual specific medical documentation; Evaluate signs of illness or dysfunction that warrant medical or nursing intervention; Basic skills required to meet the health needs of the individual; Utilizing specialized medical equipment
• Apply knowledge of nursing practices, state and federal regulations, guidelines, and procedures to ensure compliance with home and community-based services.
• Complete and document evaluations and assessments to identify training and support needs to ensure continuity of care.
• Develop care plans, reviews medical documentation, and health tracking.
• Develop effective working relations and cooperate across teams throughout the entire case management process.
• Help individuals receiving services and their caretakers make informed decisions by acting as their advocate regarding their clinical status and treatment options.
