Job summary

Heart to Heart Hospice is seeking an experienced RN Case Manager for PRN weekend coverage in Texas. You will perform comprehensive assessments and develop individualized plans of care for patients facing terminal illness.

Requirements include an accredited nursing degree, active RN license in Texas, a valid driver's license with reliable transportation, and at least two years of RN experience. This role includes on-call duties and benefits.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, psychosocial, and environmental needs.
• Implement the individualized Plan of Care (POC) and revise as necessary.
• Collaborates with patient, caregiver, physicians and other healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate care planning.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation and ensure regulatory compliance.
• Perform other duties as required.

Weekend position, primarily Saturday and Sunday with possible other PRN needs

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
