Job summary

Heart in Nashville, TN is seeking an RN Case Manager for a full-time role offering a $10,000 sign-on bonus. You will conduct comprehensive assessments and coordinate care across disciplines to meet the needs of patients with terminal illness.

Responsibilities include implementing the care plan, collaborating with physicians and care teams, attending IDG meetings, ensuring timely documentation, and maintaining compliance with regulations and policies.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patients' physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise plan as necessary.
• Collaborates care among patient/patient caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure overall compliance with laws, Medicare regulations and personnel policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
