Job summary

Heart to Heart Hospice in Indiana is seeking a dedicated RN Case Manager to coordinate comprehensive hospice care for patients and families. You will evaluate clinical needs, develop and monitor the plan of care, and ensure smooth communication among the care team.

Qualifications include an accredited nursing degree, an active RN license, a valid driver's license with reliable transportation, and at least two years of RN experience, preferably in healthcare settings.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of RN experience, preferably in a healthcare/clinical setting

Responsibilities

• Initial and ongoing comprehensive assessments of the patient's health status and needs.
• Implement and update the individual plan of care and revise the plan as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate care planning.
• Provide on-call responsibilities and on-call service to patients and families.
• Timely submission of all documentation.
• Ensure compliance with local, state and federal laws, Medicare regulations, and policies.

What You Must Have:

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
