Job summary

Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, direct and provide hospice care in home and community settings. The role emphasizes community health education and coordination with families.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse; CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice.
• Must have a valid driver’s license with an insured vehicle.
• Excellent observation, verbal and written communication; nursing skills per competency checklist.
• Able to lift, position and transfer patients; lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care with infection control measures that protect staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call responsibilities as assigned.

Position Summary:

The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience.

The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Hours / Schedule:

Full-Time Weekend Coverage.

Minimum Qualifications:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse Current CPR certification
• Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Able to lift, position and/or transfer patients.
• Able to lift supplies and equipment.

Key Responsibilities:

• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Responsible for supervising hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals and initiatives.
