Qualifications

• Graduate from an accredited nursing program (Diploma, ADN or BSN).
• Current state RN license.
• BLS certification from AHA or Red Cross.
• Three years of recent RN clinical experience.

Responsibilities

• Ensure compliance with accrediting bodies and CMS Conditions of Participation.
• Promote interprofessional coordination throughout the acute episode.
• Complete effective handoffs to the next level of care or community.
• Assess and mitigate barriers to transitions of care.
• Develop a comprehensive, individualized transitional care plan with patient/catient partners.
• Participate in patient rounding, huddles and committee meetings.
