Job summary

HCA Florida Aventura Hospital seeks a Registered Nurse Case Manager PRN to support patients and families through the care journey. You will coordinate services, develop care plans, and work with physicians and the care team to ensure safe transitions.

This position emphasizes compassionate, patient-centered care in a hospital setting with opportunities to engage in discharge planning, education, and collaboration across departments.

Qualifications

• RN license in FL is required.
• BCLS must be obtained within 30 days of start date.
• Associate Degree in Nursing; Bachelor's degree preferred.
• Experience in acute care or case management.

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients and caregivers to support treatment planning.
• Develop individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and document findings for the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote values of compassion, respect, and excellence in interactions.
