Job summary

HCA Florida St. Lucie Hospital in Port St. Lucie, FL is seeking a Registered Nurse Case Manager to guide patients and families through care transitions and coordinate services across the continuum of care.

You will collaborate with physicians, nurses, and the rehab team to develop individualized treatment plans and ensure safe, patient-centered care. This PRN role offers opportunities for professional growth through HCA’s education and certification support programs.

Qualifications

• 1 year of relevant experience
• Basic Cardiac Life Support within 30 days of start date

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning
• Develop and implement individualized treatment plans reflecting patient strengths and goals
• Complete psychosocial assessments and discharge planning evaluations, documenting findings
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care
• Promote HCA Healthcare values in every patient interaction

Education

RN license (FL)

Associate Degree in Nursing

Bachelor’s degree preferred
