Qualifications

• No travel required.
• Two years of experience in case management or related field preferred.

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized care plans reflecting patient needs and goals.
• Document psychosocial assessments and communicate needs across care teams.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless care.
• Promote HCA Healthcare values in every interaction with patients and families.
