Job summary

Valley Regional Medical Center in Brownsville, TX, part of HCA Healthcare, seeks a Registered Nurse Case Manager PRN to support patients and families through care transitions. You will coordinate services, develop care plans, and work with physicians, nurses, and leaders to ensure quality care and clear communication.

The role requires RN with TX license, BLS, and at least two years of nursing experience; case management certification is preferred.

Qualifications

• Registered Nurse with current Texas license; bachelor's degree preferred
• Ability to take call evenings, nights and weekends
• Ability to collect, interpret and report data
• Demonstrated ability to perform basic word processing and spreadsheet functions
• Demonstrated ability to think critically, analyze, and make decisions
• Demonstrated ability to communicate effectively in oral, written, and electronic form
• Demonstrated ability to establish collaborative working relationships
• Case Management or Utilization Review certification preferred
• American Heart Association (Health Care Provider) BLS required
• Two years of clinical nursing experience required; Case Management or Utilization Review experience preferred

Responsibilities

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction
