Valley Regional Medical Center in Brownsville, TX, part of HCA Healthcare, is seeking a Registered Nurse Case Manager PRN to support patients and families through care transitions. You will coordinate services, develop individualized care plans, and collaborate with physicians and the care team to ensure safe, high-quality outcomes.

Responsibilities include guiding patients through program orientation, discharge planning, and education, with emphasis on patient rights and benefits.

Qualifications

• RN with Texas license (license required)
• Ability to take call evenings, nights and weekends
• Demonstrated ability to interpret data and report findings
• Strong written and oral communication skills
• Ability to build collaborative relationships with care team
• Case Management or Utilization Review experience preferred
• Two years of clinical nursing experience
• BLS certification required
