HCA Healthcare, Inc. seeks a PRN RN Case Manager in Arlington, TX to influence patient care and coordinate care transitions.

This role covers day shifts with weekend coverage as needed and emphasizes collaboration with the multidisciplinary team to promote patient-centered outcomes. Responsibilities include comprehensive assessments, discharge planning, and resource coordination, with emphasis on safe transitions and adherence to InterQual criteria.

Formación

• Registered Nurse with current TX state license required.
• Associate Degree in Nursing or Nursing diploma required; Bachelor's Degree in Nursing preferred.
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required.
• Certification in case management preferred.
• InterQual experience preferred.

Responsabilidades

• Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
• Reassesses the patient’s clinical condition as indicated. Considers readmission risk and develops mitigation strategies including education and community resources.
• Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and physician to facilitate a successful care transition.
• In partnership with Social Services, the RN CM ensures post-acute medical needs and level of care are appropriate.
• The RN CM makes timely referrals to Social Services when psychosocial risk factors are identified.
• Involves patient and family in identifying needs and goals to develop mutual, realistic expectations.
• Evaluates progression of care using InterQual criteria; escalates issues as needed.
• Makes referrals to third party payer, disease and case management programs for recurring patients.
• Facilitates patient throughput with an ongoing focus on care transition, quality and efficiency.
• Documents recommendations, discharge plans, and care coordination activities to communicate with the health care team.
• Aligns patient needs with available resources to ensure a safe discharge/transition.
