Moments Hospice in Missouri seeks an RN Case Manager to lead clinical care for patients and families across the hospice journey. You will manage symptoms, coordinate care plans, and provide guidance during the end-of-life process.

You will work in a supportive team with manageable caseloads, access to leadership, and real opportunities to shape your career within a growing organization. Fleet vehicle program supports business and personal use.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
