Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical coordination for a caseload of patients and families in a field-based setting. You will oversee symptom management, discharge planning, and family education to ensure comfort and dignity at end of life.

The role emphasizes collaboration with physicians and the care team, with opportunities for leadership and professional growth. A valid RN license and EMR proficiency are required.

Qualifications

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
