Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for hospice patients and families. You will assess needs, coordinate care, and manage symptoms with empathy and expertise in end-of-life care.

In this full-time, field-based role, you’ll work with a supportive team, maintain reasonable caseloads, and have access to leadership. A state RN license and EMR proficiency are required.

Qualificações

Responsabilidades

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
