Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in a field-based role across Minnesota. You will assess needs, develop plans of care, manage symptoms, and coordinate cross-disciplinary services to ensure comfort and dignity at end of life.

You will educate families on disease progression, medications, and care expectations while maintaining compassionate, patient-centered care.

Qualificações

• Active Registered Nurse (RN) license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsabilidades

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and act as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
