Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in Wisconsin. You will assess needs, plan and coordinate care, and provide skilled nursing support focused on comfort and dignity at end of life.

The role offers a full-time, field-based schedule with a competitive salary and a fleet car program for business and personal use. You will collaborate with the care team to ensure holistic, patient-centered care and guidance for families.

Qualifikationen

• Active RN license in the applicable state (WI) or ability to obtain.
• Excellent organizational and communication skills.
• Ability to coordinate care and provide end-of-life support with families.

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
