Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients in need of hospice services. You will assess symptoms, coordinate plans of care with families, and ensure compassionate end-of-life care.

This role features a Monday–Friday schedule, a strong pay band, and a fleet car program for business and personal use. You will work with a supportive team that values patient-centered care and professional growth.

Qualifications

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• Compassionate, patient- and family-centered approach

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
